*I have read the vendor policies & procedures and agree to adhere to them. A copy of my certificate of insurance in a minimum amount of

$1,000,000 per occurrence under general liability is enclosed. I agree to hold blameless the Town of Lisbon for any damage or loss

associated with my display. I waive all liability that the Town of Lisbon may have arising out of my participation at the Moxie Festival. I

understand that I will be responsible for set up and removal of both my display and my trash.

*I acknowledge that submitting this application does not automatically guarantee that I will be approved as a vendor in the Moxie Festival. I understand that my application must be reviewed first, and if it is approved, I will be contacted and sent an invoice with the total amount due to reserve my space(s).
